Questionnaire logo: border settings

Added a setting to show or hide the questionnaire logo.

Attached are the examples of how it will look with the enabled and disabled border.

Warning: ability to send emails

Due to the recent spam attack, we are not able to send emails until our email provider resolves the issue at their side.
