Understanding World Sickle Cell Anemia Day: Symptoms, Causes, and Prevention

June 19 marks *World Sickle Cell Anemia Day*, a global initiative to raise awareness about sickle cell disease (SCD), combat stigma, and advocate for equitable healthcare. The 2025 theme, "Global Action, Local Impact: Empowering Communities for Effective Self-Advocacy", highlights the need for community centered solutions.
This theme focuses on uniting local communities and the global community to support sickle cell disease patients. It inspires locals to raise their voices, raise awareness, and demand better assistance and care. The goal is to strengthen connections, exchange information, and make sure sickle cell disease patients have the tools and support they need to improve their quality of life while contributing back to their communities.

What is Sickle Cell Disease?

SCD is a genetic blood disorder where abnormal hemoglobin causes red blood cells to become rigid, sticky, and crescent shaped (sickled). These cells block blood flow, leading to tissue damage, pain, and organ failure.

Sickle cell anaemia affects millions around the world.It is hereditary not contagious and it's our duty to raise our voice in creating the much needed awareness in combating the disease.

What are the Symptoms and Complications with SCD? Symptoms typically appear by age 6 months and vary in severity

1. Pain Crises - Sudden, severe pain in chest, back, joints, or limbs due to blocked blood flow. May last hours to weeks.

2. Infections - Splenic damage increases risk of pneumonia, meningitis. A leading cause of death in children.

3. Acute Chest Syndrome - Life-threatening lung blockage causing fever, cough, and hypoxia. Requires emergency care.

The Chronic Complications of SCD includes;
1. Anemia - Fatigue, paleness, and shortness of breath from rapid red cell destruction.

2. Organ Damage - Kidney failure, liver impairment, and vision loss from chronic oxygen deprivation.

3. Stroke - Blocked brain vessels cause paralysis or cognitive issues (affects 10% of children with SCD).

4. Priapism - Painful, prolonged erections leading to impotence if untreated .

Causes and Genetics

1. Inheritance Pattern - SCD requires two mutated hemoglobin genes (one from each parent). If only one gene is inherited, the person has *Sickle cell trait*(usually asymptomatic but a carrier).

2. Genetic Mutation- A defect in the *HBB* gene replaces glutamic acid with valine in hemoglobin, causing cells to sickle under stress.

3. Risk Factors- Higher prevalence in regions with malaria history (e.g., sub-Saharan Africa, India) because sickle trait confers malaria resistance .

Prevention and Management

While SCD cannot be prevented, complications can be reduced with the following Medical Strategies

1. Newborn Screening - Early diagnosis via heel-prick tests enables prompt treatment.

2. Vaccinations & Antibiotics - Daily penicillin until age 5 and routine vaccines prevent infections.

3. Medications - Reduces pain crises and acute chest syndrome.

4. Crizanlizumab - Prevents blood vessel blockages .

5. Voxelotor - Inhibits red cell sickling .

6. Curative Options (Stem Cell Transplant ) - High-risk but curative for severe cases (only 18% find compatible donors).

7. Gene Therapy - New CRISPR-based treatments (e.g., Casgevy) correct the defective gene .

Lifestyle Adjustments

1. Hydration - Drink 8–10 glasses of water daily to prevent sickling .

2. Avoid Triggers - Extreme temperatures, stress, alcohol, and high altitudes.

3. Pain Management - NSAIDs, opioids, and heat therapy for crises.

4. Genetic Counseling- Carrier screening (via blood test or prenatal testing) helps assess pregnancy risks .

Global Impact and Progress-

1. Disparities- Higher mortality in low-resource regions due to limited screening/treatment.

2. Advocacy - Events like "Shine the Light" campaigns and community health days (e.g., Juneteenth block parties) amplify education and support.

3. Hope - Improved therapies have increased life expectancy to >50 years in high-income countries.

1. Counselling - Share facts and educate intending couples on the dangers incompatible genotype.

2. Donate - Support organizations for local awareness.

3. Advocate - Push for policy changes improving SCD care access .

1- Avoid Giving your child everything he asks for. He will grow up believing that he has the right to get everything he wants.

2-Avoid laughing when your child speaks insulting words. He will grow up thinking that disrespect is entertainment.

3-Avoid remaining insensitive to bad behavior that he can display without scolding him for his bad behavior. He will grow up thinking that there are no rules in society.

4- Avoid picking up anything that your child messes up. He will grow up believing that others must take responsibility for his responsibilities.

5- Avoid letting him watch any program on TV. He will grow up thinking that there is no difference between being a child and being an adult.

6- Avoid giving your child all the money he asks for. He will grow up thinking that getting money is easy and will not hesitate to steal for it.

7- Always avoid putting yourself on his side when he is wrong against the neighbors, his teachers, the police. He will grow up thinking that everything he does is right, it is the others who are wrong.

8- Avoid leaving him alone at home when you go to the place of worship, otherwise he will grow up thinking that God does not exist.
