When should you actually start low-dose atropine in a child with myopia?

Myopia is rising fast, and projections suggest nearly half the world could be short-sighted by 2050. Atropine eye drops can slow that progression, but they are not for every myopic child, and starting them is a clinical decision, not a reflex.

This carousel lays out five evidence-based reasons to begin, and how to choose the strength.

WHEN TO START
1. Confirmed progression: spherical equivalent worsening by 0.50 D or more over a year.
2. Fast axial growth: the eye elongating by about 0.20 mm a year or more.
3. Young age at onset: the earlier myopia appears, the higher the lifetime risk, and the stronger the case to treat early and firmly.
4. Pre-myopia with a low hyperopic reserve in a young child.
5. A trajectory heading toward high myopia, where the real aim is protecting the retina, not just the spectacle number.

0.05% OR 0.01%?
0.05% gives the most consistent effect and is preferred for fast progressors and younger children. 0.01% is gentler and better tolerated, but its effect is smaller and less consistent. Match the strength to the child, not the other way round.

A few honest caveats. The numbers above are pragmatic guides, not rigid cut-offs. Atropine for myopia remains off-label in most countries, though the EU approved a low-dose formulation (Ryjunea) in 2025. Whatever the choice, pair it with axial-length monitoring and counsel families on glare, near blur, and the small chance of rebound after stopping.

For parents: the simplest protections cost nothing. More outdoor time, sensible screen habits, and an eye check every year.

काशी में दृष्टि लौटाने का काम बहुत पुराना है।

इसी सप्ताह, जिन ऋषि ने सबसे पहले इस कार्य का वर्णन किया था, उन्हें सात समंदर पार कांसे में अमर कर दिया गया।

शुक्रवार, 19 जून 2026 को महर्षि सुश्रुत की 90 किलोग्राम की कांस्य प्रतिमा का अनावरण रॉयल कॉलेज ऑफ सर्जन्स ऑफ एडिनबरा में हुआ। 1505 में स्थापित यह संस्थान विश्व की सबसे प्राचीन शल्य संस्थाओं में से एक है। प्रतिमा अब प्लेफेयर ऑडिटोरियम में स्थापित है, जिसका अनावरण एडिनबरा में भारत के महावाणिज्यदूत सिद्धार्थ मलिक और कॉलेज की अध्यक्ष प्रोफेसर क्लेयर मैकनॉट ने किया।

इस पहल का नेतृत्व तेलुगु मूल के ब्रिटिश शल्य चिकित्सक प्रोफेसर चंद्र चेरुवु ने किया, जिनके परिवार ने आंध्र प्रदेश के अपने गाँव में छह शताब्दियों से भी अधिक समय तक चिकित्सा सेवा दी है। प्रतिमा चेरुवु फैमिली फाउंडेशन द्वारा भेंट की गई और इसे तमिलनाडु के कुशल शिल्पकारों ने प्राचीन मधुच्छिष्ट (lost-wax) विधि से ढाला है। इसी दिन प्रोफेसर चेरुवु ने एक पुस्तक "महर्षि सुश्रुत: ए कम्पेंडियम, फादर ऑफ सर्जरी" का विमोचन भी किया, जिसमें आयुर्वेद और आधुनिक चिकित्सा के 36 विशेषज्ञों का योगदान है।

सुश्रुत संहिता में सुश्रुत को काशीराज दिवोदास (धन्वन्तरि) का शिष्य बताया गया है। इन्हीं पन्नों में वे राइनोप्लास्टी का वर्णन करते हैं, लगभग 120 शल्य उपकरणों का उल्लेख करते हैं, और मोतियाबिंद शल्यक्रिया का सबसे प्राचीन ज्ञात वर्णन प्रस्तुत करते हैं, अर्थात धुंधले लेंस को हटाकर दृष्टि लौटाने की विधि। उन्होंने शल्य प्रशिक्षण और आचरण के सिद्धांत उस युग में स्थापित किए, जब विश्व में कहीं और ऐसे मानक औपचारिक रूप से नहीं बने थे।

जिस नगरी में यह विद्या सर्वप्रथम लिखी गई, उसी काशी में यह कार्य करना अत्यंत विनम्र कर देने वाला अनुभव है। उपकरण पहचान में न आने की हद तक बदल गए हैं, पर भावना वही है, धुंधली आँख से परदा हटाकर किसी को फिर से प्रकाश लौटाना। आज विश्व में कहीं भी जो शल्य चिकित्सक यह कार्य करता है, वह उसी प्राचीन नींव पर खड़ा है।

इस शुक्रवार के बाद से, एडिनबरा में अपनी उपाधि लेने सीढ़ियाँ चढ़ता हर युवा सर्जन उनकी दृष्टि के नीचे से गुज़रेगा। यह स्मरण कि आधुनिक ऑपरेशन थिएटर की सूक्ष्मता की कल्पना सबसे पहले गंगा के तट पर की गई थी।

In Kashi, the work of restoring sight is very old.

This week, the sage who is said to have first described that work was honoured in bronze, an ocean away.

On Friday, 19 June 2026, a 90 kg bronze statue of Maharshi Sushruta was unveiled at the Royal College of Surgeons of Edinburgh, founded in 1505 and one of the oldest surgical institutions in the world. It now stands in the Playfair Auditorium, unveiled by India's Consul General in Edinburgh, Siddharth Malik, and the College President, Professor Clare McNaught.

The initiative was led by Professor Chandra Cheruvu, a UK-based surgeon of Telugu origin, whose family has offered healing in their village in Andhra Pradesh for more than six centuries. The statue was donated by the Cheruvu Family Foundation and cast in bronze by master artisans in Tamil Nadu using the ancient lost-wax method. On the same day, Professor Cheruvu released a companion volume, "Maharshi Sushruta: A Compendium, Father of Surgery," bringing together 36 experts in Ayurveda and modern medicine.

The Sushruta Samhita places Sushruta as a student of Divodasa (Dhanvantari), the king of Kashi. In these pages he describes rhinoplasty, sets out roughly 120 surgical instruments, and records the earliest known account of cataract surgery, the couching of the clouded lens to restore sight. He laid down principles of surgical training and conduct centuries before such standards were formalised elsewhere.

It is humbling to do this work in the same city where it was first written down. The instruments have changed beyond recognition. The intention has not: to lift the veil from a clouded eye and return a person to light. Every surgeon who does this today, anywhere in the world, stands on a foundation laid long ago.

From this Friday onward, every young fellow climbing the staircase at Edinburgh to receive their diploma will pass beneath his gaze. A reminder that the precision of the modern operating theatre was first imagined on the banks of the Ganga.
