Have you checked your email communication ...any courrier from insurance company..
any message .....

First cross query yourself did i provided correct Email id, Phone no. and home or communication address's to insurance company if no then immediately call to toll free no. Or dedicated line with details of Policy no. and member id and get updated with status.

If yes ,then timely communication about status are sent from insurance company and you are aware of what is happening with your claim.

Status may be in query, rejected ,paid or investigated.

Claim form in health insurance

Claim is one of the manadatory document which has section A B C to be filled by customer, Hospital.
Each detail in claim form is important from Personal details to ailment details and previous history, Claimed amount, hospital and doctor details

It helps for easy assessment of claim as assessor get more clarity

List of documents mandated for submission of your claim.

1.Claim form

2.Cancelled cheque with correct name of person and along with IFSC and account no.
Remember that name should match with name in the policy copy.
Insurance companies always pay amount in the name main member in the policy accordingly submit cancelled cheque.
3.Original discharge summary with full details of treatment done.
4.Original Final bills with cost wise date wise and breakup of each item to give correct clarity
5.Original investigation reports,pharmacy bills
Name on pharmacy bills and investigation should be of admitted patient.
I'd proof like Pancard, Voter I'd aadhar card passport etc

Name on all documents should be same as of I'd proof and submitted ID proof Details should be as per policy copy.
6. Original payment receipts.

Non financial documents which are discharge summary,Indoor case papers, Investigation and Pharmacy document has to be original but in case they are lost insurance companies do accept photocopy well stamped and authorized by issuing entity.
