A 39 years old male patients known case of chronic kidney disease with PERM/ tunnelled dialysis catheter in situ, presented with sepsis and multiorgan dysfunction in the form of acute liver injury, coagulopathy, encephalopathy, respiratory failure and shock. 2D echo showed a vegetation over tricuspid valve and on HRCT thorax there are multiple septic embolic lesion in lung.
Impression: acute infective endocarditis related to indwelling intravascular catheter..

Young male from Mumbai came for job to gift city, just one week of his job and then had acute febrile illness diagnosed with dengue fever and then worsened with acute severe liver injury, AKI, Encephalopathy and ARDS required mechanical ventilation. Managed in ICU and gradually recovered and discharged after around 20 days of hospitalisation. Again feeling delighted..
never underestimate a dengue fever.. 🥵🙏
