Client intake form - did you know that we have an easy online form which can be filled in ahead of your appointment?
This not only saves time in clinic allowing us to give every minute towards your treatment, but also gives you time to think about what you’d like to focus on most during your appointment and the convenience of filling it out in your own home ahead of time. That way it can be easier to make a note of any medications or things that are important for us to know about.
Equally, if you’re not comfortable with technology and would rather we take your medical history during your first consultation, that’s absolutely fine too.
Our main focus is on getting a full person of each persons health, and looking at all the factors as a whole picture.
