This one is a favourite. It’s a win all around.

So many pluses. As a dentist nothing makes me happier than bringing a patient on a journey to good oral health. The starting pictures have so many health issues going on, there’s loads of plaque and calculus, his gums are red and puffy, he had a few cavities… this profile in a young patient is on a path to dental failure. Already missing some posterior teeth. No more, not on my watch!

His concern was the position of his teeth, he didn’t know about all the other issues. This is always the way, people don’t neglect their teeth on purpose, they just don’t know their doing it. It’s the dental teams job to educate and support our patients to oral health.
After A LOT of lectures, encouragement, (nagging😅) about oral hygiene at every single appointment. I think the picture looks so much better. I may have added in a few threats too… (No Invisalign if you don’t take me seriously 😬).

The alignment looks amazing, the patient can’t stop smiling and he is now “dentally aware”.

Good oral hygiene is 70:30, the dental team can clean your teeth and remove calculus and set you up but it’s what you do on a daily basis that determines success and it’s constant, there is no end point or breaks. Every single teeth cleaning session every day is important.

Don’t forget the basics, it’s not sexy but without them, everything is failure.

Massive composites, what do you think? Yes/no?
Ideal is not always real life so we’ve got to be pragmatic.
Follow the manufacturers guidelines for every product you use. Optibond FL when studied has the best bond strength to dentine but it is also noted in all the literature that it is technique sensitive. What does that even mean? Which step do people mess up? Which part is most critical? I don’t know, so I just do what I’m told and follow the instructions.
Timing, wet/dry, light curing - depths/strengths/time.

Never fail to be surprised by these ones, at the start you’re deciding if it’s ready to cut, has the decay progressed enough to justify. Then when you open it up, it mushrooms out. Initially I wasn’t going to touch the 4 but changed my mind once I started the 5.
