RISE+ is our project that supports patients in hospital on discharge and is a service that includes visiting patients on the wards, preparing their homes for discharge and providing an on-going visiting programme

Salary: £31,200
37.5 hrs a week (flexible)

37 days A/L including Bank Holidays
