We are inviting patients of Monifieth Medical Practice to complete our short 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗔𝗰𝗰𝗲𝘀𝘀 𝗮𝗻𝗱 𝗦𝗲𝗿𝘃𝗶𝗰𝗲 𝗔𝘄𝗮𝗿𝗲𝗻𝗲𝘀𝘀 𝗦𝘂𝗿𝘃𝗲𝘆.

The survey aims to help us better understand how patients access healthcare services, awareness of alternative services available without seeing a GP, and preferences when seeking care.

Your feedback will help inform future service planning, patient education and how we direct patients to the most appropriate healthcare professional.
