The relationship of the posteroinferior comer of the slipped vertebra to the quadrants of the adjacent end plate determines the degree of spondylolisthesis. Complete forward and downward slippage of the vertebra (spondyloptosis) produces a classic "inverted Napoleon's hat" sign.
