ππ βWe canβt compare apples to oranges. Or May be we can!!!"
Propensity Score Matching (PSM)
---
β οΈ **The core problem: Selection Bias**
In real-world studies, treatment groups are often different *before treatment even starts.*
Example:
π New treatment group β younger, lower-risk patients
π₯ Standard treatment group β older, sicker patients
So if outcomes differ laterβ¦
β it may NOT be because of the treatment itself.
This is the classic:
π **Apples vs Oranges problem**
---
π― **What PSM tries to do**
PSM creates *fairer comparisons* by matching patients with similar baseline characteristics.
Think of it as:
π§ββοΈ Matching each treated patient
with
π§ββοΈ a similar control patient
based on:
β age
β s*x
β comorbidities
β disease severity
β other baseline variables
---
π **The 4-step idea behind PSM**
1οΈβ£ Collect baseline characteristics
2οΈβ£ Calculate a βpropensity scoreβ
β probability of receiving the treatment
3οΈβ£ Match patients with similar scores
4οΈβ£ Compare balanced groups
βοΈ After matching, groups become more comparable β less biased analysis
---
π‘ Important insight:
PSM does **NOT** create perfect randomization.
It only balances:
β measured variables
But:
β hidden or unmeasured confounders may still exist
Thatβs why:
π§ͺ RCTs remain the gold standard
π But PSM greatly improves observational studies
---
π Bottom line:
Without adjustment:
π β π
With proper matching:
π π€ π

Observational study designs made simple π
This infographic breaks down the core analytical designs:
πΉ Case-Control β looking from present to past
πΉ Cohort β following from present to future
πΉ Cross-Sectional β a snapshot at one point in time
πΉ Hybrid designs (Nested case-control & Case-crossover)
π‘ Each design serves a different purpose β choosing the right one is critical for generating valid evidence.
If you work with real-world data or are planning a research project, understanding these designs is essential.
