🔩 A file has fractured. What should you do next?

For many clinicians, the first reaction is: “I have to remove it.”

But that’s where the most common mistake begins.

Removing the fragment is not the primary goal. The goal is to preserve the tooth while achieving the best possible disinfection of the root canal system. In many cases, attempting retrieval causes more harm than the separated instrument itself-excessive dentin removal, perforation, or unnecessary weakening of the root.

A proper decision-making process doesn’t start with ultrasonics.

It starts with four simple questions:

• Can the fragment be bypassed?
• Where is it located?
• Is straight-line access possible?
• Does the potential benefit outweigh the biological cost?
