In a population-based longitudinal cohort of cognitively normal older adults, poorer speech-in-noise performance—a measure of central auditory processing—was associated with faster cortical thinning in temporoparietal regions involved in speech comprehension. Peripheral hearing loss and hearing aid use were not significantly associated with longitudinal neurostructural decline or cognitive change.

These findings suggest that impaired speech-in-noise ability may serve as an early behavioral marker of neural vulnerability and may precede detectable cognitive decline in aging populations.
