Without CBCT and microscope, highly complex root canal anatomies are often missed.
Tooth 46 retreatment - large apical lesion was present on D root and smaller apical lesion was present on M root. Upon taking CBCT, previously missed middle-D canal was noted. All three D roots had separate portals of exit.
