Going blank on exams is a version of " startle response".
When one of these hits there is a dump of adrenaline. If each question you read produces one of these dumps it can be enough to shut down the logical left hemisphere which is where the information needed for the exam is stored.
There is a way to prevent these startle responses. I teach these to students to allow them to succeed.
When I university I failed out two years in a row from these blanking outs.

NLP is about modelling excellence.
Choosing to do something.
Find some one who does it already in an excellent way
Get to spend as much time around them as possible while they are doing this thing excellently.
Refrain from asking process questions unless absolutely necessary.
They may be in a state of flowj...so just observe and listen.
Save the questions for " non-excellent times.
Once someone becomes unconsciously competent it may no longer be possible to retrieve the essential information to answer the question.
