The peptide regulatory landscape in 2026 is in active flux. After the FDA placed 19 popular peptides in Category 2 in late 2023, HHS Secretary RFK Jr. announced in February 2026 that approximately 14 would be returned to compoundable status, with a key FDA committee review scheduled for July 2026. Five peptides currently hold full FDA approval - semaglutide, tirzepatide, tesamorelin, sermorelin (since withdrawn), and PT-141. The rest sit at varying points across compoundable access, regulatory limbo, and active investigation.

What separates approved peptides from unapproved ones is rarely the strength of the science. It’s patentability. Naturally occurring peptides like BPC-157, MOTS-c, GHK-Cu, and TB-500 can’t be patented - so no company will fund the $500M-$2B in trials the FDA requires. Engineered molecules like semaglutide, tirzepatide, and retatrutide secure exclusivity, fund the trials, and unlock expanding labels across diabetes, obesity, cardiovascular, kidney, and liver disease. Tesamorelin is approved only for HIV despite a mechanism that applies far more broadly, because expanding the label means new trials the sponsor hasn’t run. The result is a category where regulatory status reflects economic structure more than scientific consensus.
