Un énorme hématome 🐴

Ce cheval a développé un volumineux hématome à l'intérieur du membre postérieur après avoir reçu un coup de pied d'un compagnon de pré. Ce type de blessure est relativement fréquent chez les chevaux vivant en groupe. Même si la vie en troupeau présente de nombreux avantages pour leur bien-être, les coups de pied occasionnels font malheureusement partie de leur comportement social.

Un hématome est une accumulation localisée de sang en dehors des vaisseaux sanguins, généralement causée par un traumatisme contondant, comme un coup de pied. Le choc provoque la rupture de petits vaisseaux sous la peau, ce qui permet au sang de s'accumuler dans les tissus environnants. Une poche remplie de sang et de liquide se forme alors, et elle peut continuer à augmenter de volume dans les heures qui suivent le traumatisme. Contrairement à un simple bleu, un hématome peut devenir très important, douloureux et gêner les mouvements du cheval. Sans traitement, il peut également s'infecter ou s'entourer d'une capsule fibreuse, rendant sa résorption naturelle beaucoup plus difficile.

Notre équipe a d'abord drainé le sang et le liquide accumulés afin de diminuer la pression sur les tissus et de réduire le risque de complications. Une fois la cavité décompressée, la capsule fibreuse qui s'était formée autour de l'hématome a été soigneusement retirée en curetant délicatement la paroi interne. Cette étape favorise une cicatrisation saine et diminue fortement le risque que la poche se remplisse à nouveau.

Grâce à une prise en charge vétérinaire adaptée, à des soins locaux rigoureux et à une période de repos suffisante, les chevaux récupèrent généralement très bien de ce type de blessure et peuvent reprendre progressivement leurs activités normales.

A Giant Lump 🐴

This horse developed a large hematoma on the inside of the hind leg after being kicked by a pasture mate an injury that is relatively common in horses living together in groups. While social housing has many welfare benefits, occasional kicks and bumps are unfortunately part of horse behavior.

A hematoma is a localized collection of blood that forms outside the blood vessels, usually after blunt trauma such as a kick. The impact damages small blood vessels beneath the skin, allowing blood to leak into the surrounding tissues. This creates a swollen, fluid-filled pocket that can continue to enlarge during the first hours after the injury. Unlike a simple bruise, a hematoma can become very large, painful and may interfere with movement. If left untreated, it can also become infected or develop a thick fibrous capsule, making it much more difficult for the body to absorb the fluid naturally.

In this case, our surgical team first drained the accumulated blood and fluid to relieve pressure on the surrounding tissues and reduce the risk of complications. Once the cavity was decompressed, the fibrous capsule that had formed around the hematoma was carefully removed by scraping the inside of the pocket. This stimulates healthy tissue healing and significantly reduces the chance of the hematoma filling up again.

With appropriate veterinary treatment, careful wound management and sufficient rest, horses generally recover very well from this type of injury and can safely return to their normal activities.

Een gigantische bult 🐴

Dit paard ontwikkelde een enorme hematoom aan de binnenzijde van het achterbeen nadat het een trap kreeg van een weidemaatje. Dit is een relatief vaak voorkomende blessure bij paarden die in groepshuisvesting leven. Hoewel samen op de wei veel voordelen biedt voor het welzijn van paarden, horen kleine confrontaties en een verdwaalde trap er helaas soms ook bij.

Een hematoom is een plaatselijke ophoping van bloed buiten de bloedvaten, meestal veroorzaakt door een stomp trauma, zoals een trap. Door de impact scheuren kleine bloedvaatjes onder de huid, waardoor bloed zich ophoopt in het omliggende weefsel. Zo ontstaat een met bloed en vocht gevulde zwelling die in de eerste uren na het letsel nog verder kan toenemen. In tegenstelling tot een gewone blauwe plek kan een hematoom erg groot worden, pijnlijk zijn en de beweging van het paard belemmeren. Wanneer het niet behandeld wordt, bestaat bovendien het risico op infectie of de vorming van een stevige bindweefselcapsule, waardoor het lichaam het vocht minder goed kan opruimen.

Ons team heeft eerst het opgehoopte bloed en vocht afgevoerd om de druk op het weefsel te verminderen en het risico op complicaties te verkleinen. Vervolgens werd de bindweefselcapsule die zich rond het hematoom had gevormd zorgvuldig weggekrabd. Op die manier kan het onderliggende weefsel opnieuw gezond genezen en wordt de kans dat de holte zich opnieuw met vocht vult aanzienlijk kleiner.

Met de juiste nazorg, een goede wondverzorging en voldoende rust herstellen paarden doorgaans zeer goed van dit type letsel en kunnen ze nadien hun normale activiteiten weer hervatten.

Respirer est quelque chose pour lequel aucun cheval ne devrait avoir à se battre.

Qu’il s’agisse d’un cheval de sport ou de loisir, les problèmes respiratoires peuvent fortement affecter son confort, son bien-être et sa qualité de vie.

Lors d’une récente collaboration internationale en Chine, le Dr Tom Mariën a réalisé une chirurgie de type laryngeal tie-back sur un cheval de sport atteint d’une paralysie laryngée sévère, communément appelée « roaring ».

Chez ces chevaux, le cartilage aryténoïde gauche est paralysé et ne peut plus s’ouvrir correctement pendant l’effort, ce qui limite le passage de l’air vers les poumons et impacte les performances respiratoires. Durant l’intervention, une prothèse sous forme de suture est placée afin de maintenir de façon permanente le cartilage aryténoïde en position ouverte, permettant ainsi au cheval de respirer à nouveau normalement.

Réaliser des chirurgies à travers le monde est à la fois passionnant et exigeant. Chaque clinique possède ses propres équipes, infrastructures et méthodes de travail, mais nous partageons tous le même objectif : offrir les meilleurs soins possibles à nos patients tout en continuant à apprendre les uns des autres.

Breathing is something no horse should have to fight for.

Whether in sport or leisure, respiratory problems can deeply affect a horse’s comfort, wellbeing and quality of life.

During a recent international collaboration in China, Dr. Tom Mariën performed a laryngeal tie-back surgery on a sport horse diagnosed with severe laryngeal paralysis, commonly known as roaring.

In these horses, the left arytenoid cartilage is paralysed and can no longer open properly during exercise, restricting airflow to the lungs and limiting performance. During surgery, a prosthetic suture is placed to permanently hold the arytenoid cartilage in an open position, allowing the horse to breathe again normal.

Performing surgeries around the world is both exciting and challenging. Every hospital is different, with its own teams, facilities and ways of working, but we all share the same commitment: providing the very best care for our patients and continuously learning from one another.

For Equine Care Group, these international collaborations are opportunities to share knowledge, learn from one another and continue advancing equine medicine together.

Ademen is iets waar geen enkel paard voor zou moeten hoeven vechten.

Of het nu gaat om een sportpaard of een recreatiepaard, ademhalingsproblemen kunnen een grote impact hebben op het comfort, het welzijn en de levenskwaliteit van een paard.

Tijdens een recente internationale samenwerking in China voerde dr. Tom Mariën een laryngeale tie-backoperatie uit bij een sportpaard met ernstige larynxverlamming, beter bekend als *roaring*.

Bij deze aandoening is het linker arytenoïdkraakbeen verlamd, waardoor het tijdens inspanning niet meer voldoende opent. Dit beperkt de luchtstroom naar de longen en vermindert de prestaties van het paard. Tijdens de operatie wordt een prothesehechting geplaatst die het arytenoïdkraakbeen permanent in een geopende positie houdt, zodat het paard weer normaal kan ademen.

Opereren over de hele wereld is zowel uitdagend als bijzonder inspirerend. Elk ziekenhuis heeft zijn eigen team, infrastructuur en werkwijze, maar we delen allemaal dezelfde ambitie: onze patiënten de best mogelijke zorg bieden en voortdurend van elkaar blijven leren.

Une fracture de l’ulna ne signifie pas toujours la fin

Ce cheval a été référé à notre clinique suite à une fracture de l’ulna (cubitus). Même si une fracture d’un os long peut sembler particulièrement inquiétante chez le cheval, celle-ci fait heureusement partie des fractures dont le pronostic est souvent favorable.

La raison se trouve dans le rôle spécifique de l’ulna. Contrairement à d’autres os longs du membre, l’ulna ne supporte pas directement le poids du cheval. Sa fonction principale est de résister aux importantes forces de traction générées par le muscle triceps, indispensable à l’extension du membre antérieur.

Lors de la chirurgie, les fragments osseux sont d’abord réalignés avec précision afin de restaurer l’anatomie normale de l’os. Une plaque est ensuite placée sur la face postérieure de l’ulna afin de neutraliser les fortes tensions exercées par le triceps. Grâce à cette technique appelée ostéosynthèse par bande de tension (“tension band”), les forces de traction sont transformées en forces de compression au niveau de la fracture, favorisant ainsi une cicatrisation optimale.

Ulna fracture does not mean the end

This horse arrived at our clinic with an ulna fracture, and although a broken bone always sounds alarming, this is in fact one of the mostfavourablee fractures we can see in the long bones of the horse.

The explanation lies in the function of the ulna. Unlike most other long bones, the ulna does not bear the horse's body weight. Its main role is to absorb the powerful forces generated by the triceps muscle, which is essential for extending the leg. Because the bone carries no weight of its own, the situation is far less critical than it might first appear.

To repair the fracture, we first bring the bone fragments back into their correct position. We then apply a large plate to the back of the ulna. This plate captures and neutralises the strong pulling forces produced by the triceps and converts them into compression across the fracture site. This technique is known as a tension band repair.

With this approach the prognosis is normally very good, and these patients often go on to make a full recovery.

Een ulnafractuur betekent niet het einde

Dit paard kwam bij onze kliniek aan met een ellepijpbreuk (ulnafractuur), en hoewel een gebroken bot altijd verontrustend klinkt, is dit in feite een van de meest gunstige fracturen die we kunnen zien in de lange beenderen van het paard.

De verklaring ligt in de functie van de ulna. In tegenstelling tot de meeste andere lange beenderen draagt de ulna het lichaamsgewicht van het paard niet. Haar belangrijkste rol is het opvangen van de krachtige krachten die worden opgewekt door de tricepsspier, die essentieel is voor het strekken van het been. Omdat het bot geen gewicht draagt, is de situatie veel minder kritiek dan ze op het eerste gezicht lijkt.

Om de fractuur te herstellen, brengen we eerst de botfragmenten terug in hun juiste positie. Vervolgens plaatsen we een grote plaat aan de achterzijde van de ulna. Deze plaat vangt de sterke trekkrachten van de triceps op en neutraliseert ze, en zet ze om in compressie ter hoogte van de fractuurlijn. Deze techniek staat bekend als een tension band-osteosynthese (spanbandtechniek).

Met deze aanpak is de prognose doorgaans zeer goed, en deze patiënten herstellen vaak volledig.

Une matinée avec l’équipe de médecine interne

Chaque matin commence par une évaluation approfondie de tous les patients hospitalisés. Lors de ces tournées, nos spécialistes et internes passent en r***e chaque cas individuellement et discutent de l’évolution observée au cours des dernières 24 heures.

Aucun patient ne se ressemble. Un poulain stable peut simplement nécessiter une surveillance de routine, tandis qu’un autre peut avoir besoin de soins intensifs, de fluidothérapie intraveineuse ou d’adaptations urgentes de son traitement. Tout au long de la matinée, différents cas sont évalués les uns après les autres, chacun avec ses propres défis et priorités.

Dans cette vidéo, vous pouvez découvrir quelques exemples du travail réalisé quotidiennement par notre équipe : traitement des contractures chez les poulains nouveau-nés, prise en charge de diarrhées et de déshydratation, soins de soutien, pose ou retrait de cathéters, ou encore protection de patients vulnérables face aux maladies infectieuses.

Pour notre équipe, les tournées du matin sont bien plus qu’un simple contrôle quotidien. Elles représentent un moment essentiel pour évaluer attentivement chaque patient, coordonner les plans de traitement et s’assurer que chaque cheval et chaque poulain reçoivent l’attention individuelle et les soins professionnels dont ils ont besoin.

Morning rounds at Internal Medicine

Every morning starts with a thorough evaluation of all hospitalized patients. During these rounds, our specialists and interns review each case individually and discuss the progress made over the past 24 hours.

No two patients are the same. One stable foal may simply require routine monitoring, while another may need intensive care, intravenous fluids, or urgent adjustments to its treatment plan. Throughout the morning, different cases are assessed one after another, each with its own challenges and priorities.

In this video, you can see just a few examples. From treating contracted tendons in newborn foals, managing diarrhea and dehydration, providing supportive care, placing or removing catheters, to protecting vulnerable patients from infectious diseases.

For our team, morning rounds are much more than a routine check-up. They are an essential moment to carefully assess every patient, coordinate treatment plans, and ensure that each horse and foal receives the individual attention and professional care it needs.

Ochtendrondes op de afdeling Interne Geneeskunde

Elke ochtend begint met een grondige evaluatie van alle gehospitaliseerde patiënten. Tijdens deze rondes bekijken onze specialisten en interns elke casus individueel en bespreken ze de vooruitgang die in de afgelopen 24 uur is geboekt.

Geen twee patiënten zijn hetzelfde. Een stabiel veulen heeft misschien enkel routinematige opvolging nodig, terwijl een ander intensieve zorgen, intraveneuze vloeistoffen of dringende aanpassingen aan het behandelplan vereist. Gedurende de ochtend worden verschillende patiënten na elkaar geëvalueerd, elk met hun eigen uitdagingen en prioriteiten.

In deze video zie je slechts enkele voorbeelden. Van het behandelen van peescontracturen bij pasgeboren veulens, het aanpakken van diarree en uitdroging, het bieden van ondersteunende zorg, het plaatsen of verwijderen van katheters, tot het beschermen van kwetsbare patiënten tegen infectieziekten.

Voor ons team zijn de ochtendrondes veel meer dan een routinecontrole. Het is een essentieel moment om elke patiënt zorgvuldig te beoordelen, behandelplannen op elkaar af te stemmen en ervoor te zorgen dat elk paard en veulen de individuele aandacht en professionele zorg krijgt die het nodig heeft.
