Advance care planning helps you focus on what matters most to you in terms of your future care, health, lifestyle and finances, and share these decisions with your loved ones. It also guides your loved ones when the time comes for them to make important decisions on your behalf.
Thanks to the Older Adult Health Hub and Palliative Care Western Australia, free advance care planning workshops are available until the end of the year.

This week South Metro Health Service (SMHS) recognises and celebrates NAIDOC Week, honouring the rich histories, cultures and achievements of Aboriginal and Torres Strait Islander peoples.
This yearโs theme โ 50 Years of Deadly โ celebrates five decades of strength, resilience, leadership and cultural pride. Throughout July, weโre giving staff the opportunity to take part in a range of events to learn, reflect, connect and celebrate.
Whether attending a SMHS event, joining a community celebration, or taking time to learn more about First Nations cultures and histories, we encourage everyone to get involved ๐ค๐โค๏ธ
