Pleural effusion on ultrasound – While ultrasound alone does not allow us to distinguish an exudate from a transudate, some aspects of the sonographic appearance point to either type. 🧐🍸

But, from the pathophysiological perspective, what actually distinguishes an exudative pleural effusion from a transudative pleural effusion?

A) An increase in hydrostatic pressure causes a filtration of clear, protein-poor fluid into the pleural space.
B) An alteration of the vessel wall with increased inter-endothelial spaces, leading to protein leakage that pulls water along with it.
C) A reduction in colloid osmotic pressure from hypoalbuminemia, preventing adequate fluid retention within the vascular space.

Here's a case of a 23-year-old woman who had just returned from a long plane trip and experienced syncope shortly after her return. Her electrocardiogram (ECG) showed an incomplete right bundle branch block and T-wave abnormalities.

Her echo study revealed a dilated right ventricle and flattening of the interventricular septum, raising suspicion of pulmonary embolism (PE), especially when considering the recent long-distance journey. The subsequent CT study confirmed the suspicion, depicting a massive, bilateral pulmonary embolism, and the patient was put on systemic thrombolysis.

So now we have a confirmed diagnosis, but where did the embolus originate? 💡
