Let me tell you about the breach that didn't need to happen.A regional hospital system. Solid security posture. Good IAM hygiene for employees.But a vendor account, one that should have been deactivated 6 months earlier, became the entry point for ransomware.

How did it happen? The vendor finished their EHR upgrade project in March.

The contract ended in April.
Access was supposed to be revoked in May. It's now November. The account is still active. And it just got compromised.Sound extreme?It's not.
