Utah’s Department of Health and Human Services, Office of Licensing, revoked Provo Canyon School’s license this week.

Their own June 2026 inspection report documented altered incident reports, seven critical incidents that were never reported at all, two clients who swallowed batteries and waited days for medical care, and a su***de attempt during a supervision gap.

Staff conduct toward clients was documented directly, along with background checks skipped for staff who had access to kids.

Several of these findings are repeat citations going back years.

All services at the campus must stop by August 6.
