I talk to a lot of families who get confused about Indiana’s waiver programs. Many qualify for more support than they realize, especially under PathWays and Health and Wellness.

A simple way I explain it to families:

• If daily tasks are becoming unsafe
• If mobility is declining
• If supervision is needed
• If the family caregiver is overwhelmed

They may meet Nursing Facility Level of Care and qualify.

The long nights, the rushed breakfasts, the constant decisions. It drains your energy before you even notice it happening.

You can create small pockets of relief by doing simple things that re-center your day:

• Take five minutes to sit outside before the day starts
• Delegate one task you no longer have the capacity for
• Stop trying to fix everything at once

Even small adjustments lighten the weight you have been carrying for too long.

What does “Nursing Facility Level of Care” mean for PathWays eligibility?

“Nursing Facility Level of Care” means the person needs enough daily help that, without support, they would be at risk of entering a nursing home.

This is based on functional needs, not age and not income.

If someone needs assistance with tasks like bathing, dressing, mobility, toileting, medication management, safety supervision, or has chronic medical conditions that affect daily life, they may meet the criteria.

PathWays is designed to keep people safely at home when these needs are present.
