MAHABA YOGA LIABILITY WAIVER

Before attending a class, please read the following.
Your attending a class means you agree to the following and will serve as your signature.

1. I am participating in classes or services during which I receive information and instruction about yoga. I am aware that yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am aware of possible risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical and fitness program, including yoga, I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities.

3. I agree to assume full responsibility for any risks, injuries, known and unknown which I might incur as a result of participation. It is imperative to listen to your body during yoga practice.

4. I knowingly, voluntarily and expressly waive any claim I may have against the instructor, the owner, for any injuries as a result of participating in class.



I have read the above release and waiver of liability and understand its contents. I voluntarily agree to the terms and conditions stated above.
