𝐓𝐡𝐞 𝐂𝐨𝐦𝐦𝐢𝐬𝐬𝐢𝐨𝐧 𝐫𝐞𝐦𝐢𝐧𝐝𝐬 𝐚𝐥𝐥 𝐢𝐧𝐭𝐞𝐫𝐞𝐬𝐭𝐞𝐝 𝐚𝐩𝐩𝐥𝐢𝐜𝐚𝐧𝐭𝐬 𝐨𝐟 𝐭𝐡𝐞 𝐟𝐨𝐥𝐥𝐨𝐰𝐢𝐧𝐠:

2. The period of filing is from May 3, 2026 at 8 AM to May 7, 2026 at 8 PM

3. Using your CPU email, download the provided document via the link below for the application form. Read and follow the instructions properly.

4. Save your accomplished application form as a PDF file. Label as "SURNAME, FIRST NAME M.I.- POSITION" (ex: DELA CRUZ, JUAN J.- PRESIDENT)

5. Using your CPU email, accomplish the Google Forms via the link below. Submit your PDF file on the provided section.
