√ 2 long xerox copy of Marriage Contract if married ( at least PSA)
√ 2 long xerox copy of High School or College Diploma
√ 2 long xerox copy of Valid ID
√ 2 long xerox copy of Vaccination Card
√ Brgy Clearance
√ 2 pcs 2x2 ID picture
√ 4 pcs Passport Size ID picture with nametag ( First Name, Middle Initial, Last Name) and in business attire
