MEDICAL TAB UPLOAD INSTRUCTIONS

Please upload the following documents to the Student Portal Medical Tab. Ensure that all documents are properly scanned, clear, complete, and readable using a scanning application (e.g., CamScanner). Blurred, incomplete, or improperly scanned documents may delay the verification process.

Required Documents:

1. Complete Medical Examination Form (2 pages)
> Accomplished and signed by the attending physician, where applicable.
> Attach one (1) recent passport-size ID picture in the designated area.

2. Chest X-Ray Official Reading
> Upload the official radiologist's report.
> Ensure that the patient's name and other identifying details are clearly visible.

3. ABO Blood Typing Result
> Upload the official laboratory result indicating your blood type.

4. Patient Consent Form
> Completely filled out and signed by the student and/or parent/guardian, if applicable.

Important Reminders:
1. Upload documents in PDF format whenever possible.
2. Ensure that all pages are complete and properly oriented before uploading.
3. Verify that all information is legible and matches your personal details in the student portal.
4. Incomplete, falsified, altered, or unreadable documents will not be accepted and may require resubmission.

Please be advised that medical and dental examinations for incoming freshmen at the University Infirmary will be temporarily unavailable on June 10, 2026, as the medical and dental personnel will be performing official duties in connection with the Baccalaureate and Hooding Ceremony in the morning and the Commencement Exercises in the afternoon.

Regular medical and dental services will resume on June 11, 2026 (morning only). However, services will again be temporarily suspended in the afternoon as the medical and dental staff will be deployed to provide support during the Commencement Exercises.

ANNOUNCEMENT

Please read and follow the instructions carefully.

Students are advised to undergo their laboratory and diagnostic tests first before proceeding to the MMSU University Infirmary or to any licensed physician and dentist for their medical and dental examination. All necessary forms and links may be downloaded through the student portal.

1. Laboratory and Diagnostic Tests
Only the tests indicated in the downloaded Laboratory Request Form are required to be performed. No additional laboratory or diagnostic tests are required. If other tests are being requested, kindly inform the laboratory personnel that only the tests stated in the official laboratory request should be conducted.

2. Degree Programs Without Posted Schedule
For degree programs without a posted schedule, students may have their medical and dental examination conducted by any licensed physician and licensed dentist. This means you may proceed to any licensed doctor and dentist of your choice.

3. Submission of Original Copies of Forms
Once your medical forms have been approved in the student portal, a notice stating “You may now submit original copies” will appear. Students may submit the original copies of their forms before their registration date. Please note that the registration date refers to the enrollment schedule issued by the MMSU Registrar.

4. Psychological and Neuropsychological Tests
Psychological and neuropsychological tests are NOT required. Students may inform the physician accordingly.

5. COVID-19 Vaccination
COVID-19 vaccination is no longer required.

Please be informed that the updated schedule for the Medical and Dental Examination of incoming freshmen enrolled in degree programs required of FACE-TO-FACE at the University Clinic.

All concerned students are advised to strictly follow their assigned schedule to avoid congestion and ensure an orderly process.

Please be informed that there will be an adjustment in the schedule for Degree Programs required of Face-to-Face Medical Examination due to the declaration of a holiday on May 27, 2026, as announced by Malacañang.

All affected schedules on the said date and succeeding dates will be moved accordingly. Students are advised to wait for the updated schedule to be released by the University Clinic.
