One of the most effective root cause analysis tools in Health, Safety, and Operations is the 5 Whys Technique.

When an incident, near miss, or operational issue occurs, the first answer is rarely the real cause. By repeatedly asking "Why?", we can move beyond surface-level explanations and uncover the underlying root cause.

Why use the 5 Whys Method?

โœ…Identifies the true root cause of a problem
โœ…Prevents recurrence of incidents and failures
โœ…Improves corrective and preventive actions
โœ…Simple, practical, and highly effective
โœ…Strengthens continuous improvement and safety culture

In HSE, finding the root cause is critical. If we only address immediate causes, the same incident may happen again. When we identify and eliminate the root cause, we create lasting improvements.

Don't just fix the problem-eliminate the reason it happened.
