Chronic illnesses require consistent care, regular follow-up, and strong connections between health facilities, health workers, and communities. ποΈπ¨
The Peopleβs Voice Survey found that 60% of women reported living with a chronic illness, yet only 21% regularly follow up for care. This highlights an important opportunity to strengthen continuity of care for women across the health system.
Chronic illnesses include conditions such as Hypertension, Type 2 Diabetes, Asthma, and HIV/AIDS, which require ongoing care and regular medical follow-up.
Regular follow-up visits are essential for:
β Adjusting treatment plans
β Early detection of complications
β Preventive counseling and screening
Together, health facilities and front-line health workers can strengthen follow-up care by:
β Strengthening patient follow-up systems
β Improving appointment reminders and community outreach
β Ensuring consistent availability of medicines
β Promoting patient education on chronic illness management
β Strengthening collaboration with community health workers
Stronger follow-up systems can help ensure women receive the continuous care they need to live healthier lives.
What additional steps can health facilities and front-line health workers take to ensure more women return for follow-up care?
