Healthy Hearts is specifically designed to support individuals who are at high risk of cardiovascular disease (CVD), living with a CVD diagnosis, or managing related conditions such as high blood pressure.

What you'll get:

- Free Access to Meal Planning App

Who's eligible:
- Aged between 55-65
- Registered to: Laurel Bank Surgery, The Village Surgeries, Kelsall Medical Centre, Tarporley Health Practice, Drs Adey & Dancy Medical Centre, Bunbury Medical Practice
- Diagnosed with Hypertension

We’re planning a community event at Whitby Park on 30th July and we’re looking for local organisations, groups, and services to register their interest in having a stall 🎉

This will be an activity-focused event, so all stallholders must be able to provide some kind of interactive activity for attendees to get involved in - whether that’s games, sports, crafts, wellbeing activities, demonstrations, or something creative for families and the community.
