After some GREAT guesses, it’s time for the answer... the diagnosis was Klippel-Feil syndrome.

This patient is in their 60s and had lived with the condition their entire life without knowing it was part of a recognised syndrome. While the diagnosis will be confirmed with genetic testing, the imaging findings provided the missing piece of the puzzle and helped explain many of the symptoms experienced over the years.

A diagnosis like this isn’t just about naming a condition, it can help guide future care, identify associated health issues to monitor, and provide clarity after decades of unanswered questions.

🧩 Can you solve this case?

Here’s a fascinating case with multiple imaging clues.

A man in his 60s presents with:
- Poor renal function
- Neck pain
- Chest pain
- Generally feeling unwell

The clues:

• Failure of segmentation of the cervical spine
• An aberrant right subclavian artery
• A dilated ascending aorta
• Unilateral renal agenesis with compensatory enlargement of the remaining kidney
• An ectopic ureteric insertion with a markedly dilated ureter

All of these findings point towards a single underlying diagnosis.

Proactive health is about understanding your risk factors and taking control of what you can.” – Dr Zane Sherif

Our understanding of health and disease continues to evolve. While genetics play an important role in many conditions, they are only one part of the picture. Family history may increase your risk, but it doesn’t necessarily determine your future.

A common way to describe this is: “Genetics loads the gun, but environment pulls the trigger.”

Lifestyle and environmental factors, including smoking, va**ng, diet, exercise, sleep, and other exposures can all influence whether a genetic predisposition ultimately develops into disease.

The encouraging news is that many of these factors are modifiable.

Proactive healthcare isn’t about waiting until you’re unwell. It’s about understanding your personal risk profile, having informed conversations with your healthcare team, and using appropriate screening and investigations when clinically indicated. The goal is to identify potential health issues earlier, support prevention where possible, and empower people to make informed decisions about their long-term health.

Part 2: The Answer 👇

Firstly, a huge well done to all the people who joined the discussion on Part 1. There were some fantastic clinical thoughts and differential diagnoses in the comments. 👏

The clue was that her symptoms began after the birth of her second child via C-section.

On pelvic MRI, the diagnosis became clear: endometriosis embedded within her Caesarean section scar, sitting directly adjacent to the bladder.

Every time her bladder filled or emptied, it tugged on the surrounding scar tissue and endometriosis, explaining years of bladder urgency, frequency and chronic pelvic pain.

Despite having hip MRI and ultrasound investigations, she had never undergone a dedicated pelvic MRI.

This case is a powerful reminder that knowing where to look is just as important as what you’re looking for. The right imaging, performed for the right clinical question, can completely change a patient’s journey.

The good news? Once the diagnosis was made, treatment options became available.

Educational discussion for healthcare professionals and those interested in pelvic health: 🕵️

Chronic pelvic pain can be complex, particularly when accompanied by urinary symptoms such as urgency and frequency.

Symptoms like these may have multiple potential causes and can overlap between different conditions, sometimes leading to delays in diagnosis.

Not every ACL injury automatically means surgery.

Treatment decisions should be based on the individual, not just the injury.

For some patients, a carefully selected non-surgical management pathway may be appropriate. At MBR Health, we’re exploring the role of high-dose PRP, advanced imaging and collaborative rehabilitation as part of a multidisciplinary approach for selected ACL injuries.

Every patient is different. Factors such as the type of ACL injury, knee stability, activity level, goals and clinical assessment all play an important role in determining the most appropriate management.

Our aim is to work alongside physiotherapists and other healthcare professionals to help patients understand the treatment options that may be available to them.

If you’ve recently injured your ACL and would like to learn more, speak with your healthcare professional or contact our team to discuss whether an assessment may be appropriate.

High-dose PRP is not suitable for every patient.

A fascinating case highlighting the potential role of Spectral CT in breast imaging.

This patient, a woman in her 60s, presented with a palpable lump in her left breast. Despite having extremely dense breast tissue for her age, Spectral CT identified an area of concern that correlated precisely with the location of her symptoms.

Further assessment with ultrasound and biopsy confirmed an invasive ductal carcinoma.

Dense breast tissue can make cancer detection more challenging on conventional imaging, which is why emerging technologies such as Spectral CT are generating increasing interest as complementary tools in selected patients.

Men’s Health Week may be over, but the message remains the same: don’t ignore symptoms and don’t put off getting checked.

In this case, a gentleman in his 40s had been feeling generally unwell for around six months. His journey began with a Spectral CT scan, which identified abnormalities requiring further investigation. MRI and PET imaging provided additional information, and ultimately biopsy confirmed prostate cancer.

This case highlights how different imaging modalities can work together to help investigate symptoms and guide diagnosis.

Spectral CT is another valuable tool in the assessment of a wide range of conditions.
